Interview with oncologist-mammalogist, reconstructive surgeon Igor Mоtuzyuk

«I graduated from the Bogomolets National Medical University. I have 20 years of experience in breast reconstructive surgery. I now work at the National Cancer Institute»
Which patients visit you most often?
«Most often, patients diagnosed with breast cancer are treated. Common problems are defects after surgery elsewhere, the absence of mammary glands. Patients who, for some reason, are not satisfied with aesthetics or have developmental disorders of the mammary glands, are treated less often.»
Could you give a comparative description of breast reconstructive surgery in our country and abroad, in less developed and more developed countries?
«Today we perform the same volume of operations with the same percentage of breast reconstruction using implants and using the patient’s own tissues as the United States of America.
As for the comparative characteristics of breast reconstruction surgeries, it should be said that there are American and European beauty standards, which have some differences. In Ukraine, we can focus on both the first and the second, depending on what the patient likes more.
And you may find this surprising, but since we are less closely connected by bureaucratic issues and insurance companies, in some technical aspects (placements of implants, incision sites and suture technique), we have slightly overtaken Europe several times. Even that was.
I don’t plan to operate on Da Vinci yet. In the case when it is possible to perform a subcutaneous mastectomy (in others, it is necessary to resort to an open operation, in other words, the robot doesn’t make sense), I perform it through a 2 cm incision. The robot has only one 2-centimeter port, but at least a 3-centimeter port is needed.»
What determines the success of breast surgery?
«Almost entirely — the experience of the surgeon. The more he operates, the better the aesthetic results and fewer complications.»
What is your own experience in numbers?
«I perform about 300 breast reconstructive surgeries a year. I don’t take into account less complex operations.»
Do you take on advanced stages of breast cancer?
«Of course, we take on such cases. Now the approach in oncology has changed a bit. And patients with advanced stages undergo chemotherapy or chemoradiotherapy to reduce the tumor as much as possible, and then undergo a radical surgery. Even in such cases, simultaneous reconstruction is possible, that is, the patient lies on the operating table with her breast and wakes up from anesthesia with her breast. This is very important for women from a psychological point of view.»
Is it possible to decide on a treatment plan online, get a remote consultation, undergo chemotherapy or radiation therapy at the place of residence?
«Yes, we usually do that. I have many nonresident and foreign patients. The peculiarity of breast surgery is that in addition to the results of CT scan, biopsy and laboratory tests, it is necessary to apply a full face and profile photo of the breast. In this case, I can offer a treatment plan, operation remotely and together with the patient choose the best option.»
